New Hires
New employee owners are eligible for benefits the first day of the month following 30 days after your full-time employee date of hire.
Spouse Surcharge
Spouses who have access to a medical plan through their employer, can still be enrolled on the plan, but employees must pay a $200 surcharge per month (in addition to the regular contribution rates).
Dependents
Dependents may be covered under the medical, dental, and vision plans until the end of the month in which they have their 26th birthday. For Vol Life, Critical Illness, Hospital Indemnity and Accident, coverage ends on the dependent’s 19th birthday, unless the dependent is a full-time student, then coverage may continue to age 23.
